2) Equifax Credit Report and Score (download from equifax.ca)

3) 2 pieces of Identification (driver’s license, passport, Ontario ID card)

4) Income Confirmation

– Recent payslip(s), T4, notice of assessment (NOA), employment letter
– Recent notice of assessment; T1 general or NOAs 3 years (if self employed)

– Corporation T2s and NOAs for last 3 years and Corporation Articles (if incorporated)
